How to fill-up the form
For Household – Please fill in yours and your cats details. Leave unrelated part such as Dam and Sire. Please MARK the “household” under NON-CHAMPIONSHIP.
For Pedigree – Please fill in all the details and attach a photocopy of the cat’s pedigree certificate from the governing bodies. Breeder's certificates or acknowledgements are NOT accepted.
